• Date and time of the incident
• Person(s) involved
• Nature of the injury or near-miss
• Actions taken

This helps identify patterns, improve safety measures, and ensure proper reporting to authorities if needed.

How I do it. A QR code is next to the First Aid box.

Have an accident, cut, scrape etc.

Scan the QR code.

This will take you to a form to complete.

The form is tied to a spreadsheet for those running the show to keep track of accidents and Incidents.
